How we handle your information
Merije Chukumerije, MD Inc., doing business as The Center for Performance & Preventive Cardiology ("The Center," "we," "our," or "us"), is committed to protecting the privacy of our members and website visitors. This Privacy Policy describes how we collect, use, disclose, and safeguard information when you visit our website or interact with our practice.
Information We Collect
We collect information you provide directly through our founding member waitlist form: your name, email address, phone number, general activity level, and age range. We may also collect technical information about your website use — including IP address, browser type, device type, referring page, and pages visited — through standard website analytics.
Our website does not request, and you should not submit, medical history, symptoms, test results, or other health information. Our forms contain no free-text field for this reason.
How We Use Your Information
We use the information we collect to respond to waitlist inquiries, communicate with you about founding membership and enrollment, send updates you have asked to receive, measure the performance of our advertising, improve our services, and comply with legal obligations. We do not sell your personal information to third parties.
HIPAA and Protected Health Information
As a medical practice, The Center is subject to the Health Insurance Portability and Accountability Act (HIPAA). HIPAA protections apply to protected health information created in the course of care.
This website does not collect protected health information. Submitting the waitlist form does not establish a physician-patient relationship and does not create a medical record. Once the practice opens and you enroll as a member, you will receive our Notice of Privacy Practices, which governs how your medical information is handled. That notice is a separate document from this policy.
Cookies & Tracking Technologies
Our website uses cookies and similar tracking technologies to enhance your browsing experience, analyze website traffic patterns, and measure the performance of our advertising. Cookies are small data files placed on your device. Analytics and advertising technologies load only after you accept them in the consent banner shown on your first visit; if you decline, they are not loaded at all.
Our site is hosted on Squarespace, which sets cookies necessary for the site to operate.
Analytics
We use Google Analytics for aggregate traffic analysis. Google’s use of information collected through our site is described at How Google uses information from sites or apps that use our services.
Advertising and retargeting
We use advertising and retargeting technologies, which may include the Meta Pixel and Google advertising tags, to measure the performance of our advertising and to show relevant ads to people who have previously visited our website. These technologies may set cookies and share limited technical and behavioural information — such as pages viewed, device type, and IP address — with the relevant advertising platform.
You can manage advertising preferences through your Google Ads settings, your Meta ad preferences, or your browser’s cookie controls.
Separation of form data from tracking
Information you submit through our waitlist form — your name, email address, phone number, activity level, and age range — is stored separately and is not transmitted to Google Analytics, the Meta Pixel, or any advertising platform. Our analytics and advertising tools receive only anonymous technical and behavioural data about site usage. Personal form submissions are never pooled with, or passed as parameters to, these tools.
Spam prevention
Our waitlist form uses Google reCAPTCHA to prevent automated submissions. reCAPTCHA collects device and browser information and transmits it to Google for analysis, subject to Google’s Privacy Policy and Terms of Service.
Sharing of Information
We may share your information with our technology and service vendors operating under appropriate confidentiality agreements, and as required by law. Apart from the advertising and analytics technologies described above, we do not share your information with marketing companies, data brokers, or unaffiliated third parties for commercial purposes.
Text Messages
If you agree to receive text messages from us, we use your mobile number only to send the messages you agreed to and to reply to you. Message and data rates may apply, and message frequency varies.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text-messaging opt-in data and consent are excluded from all of the sharing described above and will not be shared with any third parties.
To stop receiving messages, reply STOP to any text. For help, reply HELP or contact us at the details below.
Data Security
We implement reasonable technical and organizational measures to protect your information from unauthorized access, use, or disclosure, including encrypted transmission and access controls. No method of data transmission or storage is 100% secure, and we cannot guarantee absolute security against all potential threats. This is why we ask that you not send health information through this website.
Your Rights
You have the right to access, correct, or request deletion of personal information we hold about you, subject to applicable legal requirements. To exercise these rights, or to be removed from the waitlist entirely, contact us using the information below.
California Privacy Rights
California residents have additional rights under the California Consumer Privacy Act, as amended by the California Privacy Rights Act, including the right to know what personal information is collected, the right to delete personal information, and the right to opt out of the sale or sharing of personal information.
We do not sell your personal information for money. However, our use of the advertising and retargeting technologies described above may constitute "sharing" personal information for cross-context behavioural advertising under California law. To opt out, decline cookies in the consent banner, enable a Global Privacy Control signal, or contact us at the details below and we will honour your request.
Changes to This Policy
We may update this Privacy Policy periodically. Material changes will be posted on this page with an updated effective date. Continued use of our website or services after changes are posted constitutes acceptance of the updated policy.
